Become a resident.  

1. Contact us by email to discuss the services Sierra Salem provides and the supports the individual may need.

2. Arrange a time to tour at least one of the homes. This provides the opportunity to ask questions and get a feel for the home environment. The discussion of services and needs is often addressed during the tour.

3. If the potential resident and their family like what they see, Sierra Salem’s Application will need to be completed:

   1. The family will ask their loved one’s service coordinator at the Valley Mountain Regional Center to provide a placement packet which needs to include the Individual Program Plan and an assessment of the individual’s needs and capabilities as well as some medical history.
